Rios Click Labs is operated by Rios Healthcare Associates, PLLC. By placing an order through Rios Click Labs, you acknowledge and agree to the following terms, notices, consents, and policies. These acknowledgements apply to the patient named in the order and, when applicable, to the patient’s parent, guardian, or legally authorized representative.
Rios Click Labs provides access to selected laboratory testing services, laboratory order coordination, payment processing, and related administrative support. Depending on the order selected, services may include laboratory test ordering, specimen collection coordination, review of laboratory orders, communication of results, and optional physician review.
Rios Click Labs is not an emergency service. Do not use Rios Click Labs for urgent symptoms, emergency symptoms, or conditions that may require immediate medical attention. If you are experiencing chest pain, shortness of breath, severe abdominal pain, weakness, confusion, severe allergic reaction, suicidal thoughts, symptoms of stroke, severe infection, uncontrolled bleeding, or any other emergency condition, call 911 or seek emergency care immediately.
By accessing or using the Rios Click Labs website, placing an order, submitting information, completing checkout, or using any Rios Click Labs service, you agree to these Terms of Service and the Rios Click Labs Consent & Policies.
Rios Click Labs is operated by Rios Healthcare Associates, PLLC. These Terms apply to the website, checkout process, laboratory order services, payment processing, consent forms, communications, optional Physician Review, and related services provided through Rios Click Labs.
Rios Click Labs is currently available only for patients located in Texas. By using the service, you confirm that the patient for whom testing is requested is physically located in Texas. Rios Click Labs may decline, cancel, or refuse any order that does not meet this requirement.
You may use Rios Click Labs only for lawful purposes and only for yourself, your minor child, or another patient for whom you have legal authority or appropriate authorization to act.
You may not submit false, misleading, fraudulent, incomplete, or unauthorized information. You may not impersonate another person, interfere with website operation, attempt unauthorized access, submit malicious code, misuse the checkout system, or use Rios Click Labs in a way that violates applicable law or the rights of others.
Rios Click Labs is not an emergency service and should not be used for urgent or emergency symptoms. If you are experiencing a medical emergency, call 911 or seek emergency care immediately.
Submission of an order does not guarantee that the order will be accepted, processed, or completed. Rios Click Labs may decline, cancel, modify, delay, or request additional information for an order when medically, operationally, legally, or administratively appropriate.
Test availability, pricing, specimen requirements, turnaround time, collection options, Physician Review availability, and service features may change without notice before purchase.
Payment is due at the time of order unless otherwise stated. By submitting payment, you authorize Rios Click Labs and its payment processor to charge the selected payment method for the order total shown at checkout.
Rios Click Labs is currently a cash-pay service unless a specific insurance-billing arrangement is offered in writing. Rios Click Labs does not guarantee insurance reimbursement.
Refunds are limited and are governed by the Refund Policy section of these Consent & Policies. All sales are generally final once an order is submitted and payment is processed, except in limited circumstances determined by Rios Click Labs or as required by law.
Rios Click Labs services do not replace emergency care, diagnosis, treatment, primary care, specialty care, urgent care, or ongoing medical management. Laboratory testing and optional Physician Review are limited services and should be interpreted in context with appropriate medical care.
If you purchase Physician Review, you agree that the service is limited to the scope described in the Optional Physician Review section of these Consent & Policies. Physician Review does not establish a comprehensive primary-care relationship and does not obligate Rios Click Labs or Rios Healthcare Associates, PLLC to provide ongoing treatment or follow-up unrelated to the selected laboratory order and results.
You agree that electronic acknowledgments, electronic consent forms, typed signatures, electronic communications, and electronic records may be used in connection with Rios Click Labs services. Your typed name and submitted acknowledgments may be treated as your electronic signature.
Rios Click Labs may use third-party services, including payment processors, laboratories, electronic health record systems, communication tools, hosting providers, and other vendors. Use of those services may be subject to additional operational requirements or third-party terms.
The Rios Click Labs website, design, text, graphics, logos, product descriptions, organization, and related content are owned by or licensed to Rios Healthcare Associates, PLLC, unless otherwise stated. You may not copy, reproduce, modify, distribute, or use website content for commercial purposes without written permission.
To the fullest extent permitted by law, Rios Click Labs and Rios Healthcare Associates, PLLC are not liable for indirect, incidental, consequential, special, exemplary, or punitive damages arising from use of the website, delay or inability to use services, laboratory delays, third-party service issues, electronic communication issues, or patient decisions made without appropriate medical care.
Nothing in these Terms is intended to limit rights or remedies that cannot be limited under applicable law.
Rios Click Labs may update these Terms of Service and related policies from time to time. The version in effect at the time of order may be documented in the order record and/or signed consent packet.
These Terms are governed by the laws of the State of Texas, except where federal law applies.

Rios Click Labs is currently available only for patients located in Texas.
By placing an order, you confirm that the patient is physically located in Texas and that the laboratory order, any optional physician review, and any related services are being requested for a Texas patient.
Orders for patients outside Texas may be cancelled or declined.
You agree to provide accurate and complete information, including the patient’s legal name, date of birth, biological sex, contact information, address, and other information requested during checkout.
You understand that inaccurate, incomplete, or misleading information may affect the accuracy of laboratory processing, clinical review, communication of results, and patient safety.
You are responsible for notifying Rios Click Labs or Rios Healthcare Associates, PLLC if any information submitted with your order is incorrect.
By submitting an order, you consent to the laboratory testing services selected during checkout.
You understand and agree that:
After payment is received, Rios Click Labs will review and process your order. Laboratory orders are not automatically transmitted to a laboratory at the moment of online purchase. Orders may require administrative review, verification, and entry into Rios Healthcare Associates, PLLC’s electronic health record or laboratory ordering system before specimen collection can occur.
Once your order is processed, you will receive instructions regarding next steps for specimen collection. Depending on the tests ordered, patient location, laboratory availability, and operational requirements, specimen collection may be available at a participating Labcorp location of your choice.
Patients in the Houston area may also be able to complete specimen collection at Rios Healthcare Associates, PLLC, located at 3899 Southwest Fwy, Houston, TX 77027, when available and appropriate for the tests ordered.
Labcorp locations, hours, appointment availability, specimen requirements, and collection capabilities may vary. Patients are responsible for following the instructions provided by Rios Click Labs, Rios Healthcare Associates, PLLC, Labcorp, or any other designated collection site.
If you purchase or receive Physician Review, you consent to a limited professional service by a Rios Healthcare provider. This limited service may include review of your selected lab order, submitted information, laboratory results, medication history if authorized, and related documentation for the purpose of providing general lab-related guidance, comments, safety considerations, and recommended follow-up.
This consent is limited to the Physician Review service purchased through Rios Click Labs. It does not establish a comprehensive primary-care relationship, does not replace a full medical visit, and does not obligate Rios Click Labs or Rios Healthcare Associates, PLLC to provide ongoing medical management, urgent care, emergency care, specialty care, medication management, diagnosis, treatment, or follow-up unrelated to the selected laboratory order and results.
You understand that a provider may recommend that you seek in-person evaluation, primary care, urgent care, emergency care, specialist care, repeat testing, or additional testing. You remain responsible for obtaining appropriate medical care.
Laboratory results may be made available through Rios Healthcare Associates, PLLC, Athena, secure electronic communication, patient portal, telephone, email notification, or other approved communication methods.
You understand that:
If results suggest a potentially urgent or serious condition, Rios Click Labs or Rios Healthcare Associates, PLLC may attempt to contact you, but you remain responsible for seeking timely medical care.
If the patient is under 18 years old, the order must be submitted by the patient’s parent, legal guardian, or other legally authorized representative.
By submitting an order for a minor, you certify that you have legal authority to consent to laboratory testing and related services for the minor patient.
Rios Click Labs may request additional information or decline an order if legal authority is unclear.
Rios Healthcare Associates, PLLC maintains a Notice of Privacy Practices describing how protected health information may be used and disclosed and how patients may exercise their privacy rights. Rios Click Labs is operated by Rios Healthcare Associates, PLLC, and information submitted through Rios Click Labs may become part of the patient’s medical record.
By submitting an order through Rios Click Labs, you acknowledge receipt of this Privacy / HIPAA Notice and understand that your medical information may be used and disclosed as permitted or required by law, including for treatment, payment, healthcare operations, laboratory ordering, specimen collection coordination, results reporting, care coordination, quality assurance, legal compliance, public health reporting, business associate services, and other healthcare-related purposes.
Information related to your order may include your name, date of birth, contact information, address, biological sex, selected laboratory tests, payment status, consents, signed authorization records, order history, laboratory requisitions, laboratory results, physician review documentation if purchased, communications, and related administrative or clinical documentation.
Rios Healthcare Associates, PLLC / Rios Click Labs may share information with laboratories, electronic health record systems, payment processors, communication vendors, hosting providers, business associates, collection sites, healthcare providers involved in your care, and other persons or entities when permitted or required by law.
Laboratory orders and results may be processed through Athena or another electronic health record or laboratory ordering system. Results and related documentation may be stored in the medical record and may be communicated through secure portal, electronic message, telephone, mail, email notification, downloadable document, or other approved communication method.
You have privacy rights under applicable law, which may include the right to request access to your medical information, request an amendment, request restrictions, request confidential communications, receive an accounting of certain disclosures, and file a privacy complaint without retaliation.
You understand that standard email and text message communications may not be fully encrypted unless sent through a secure portal or secure system. Rios Click Labs will use reasonable safeguards, but electronic communication carries some risk of unauthorized access.

By submitting an order, you consent to receive records, notices, receipts, consent forms, payment communications, order updates, and other communications electronically.
Electronic communications may include email, text message, patient portal message, electronic forms, downloadable documents, or other electronic methods.
You understand that some electronic communications may not be encrypted unless sent through a secure portal or secure system. Rios Click Labs will use reasonable safeguards, but electronic communication carries some risk of unauthorized access.
You agree that electronic signatures, electronic acknowledgments, and electronic records may be used in connection with your order.
Payment is due at the time of order unless otherwise stated.

You understand and agree that:
All sales are generally final once an order is submitted and payment is processed. Because Rios Click Labs provides laboratory ordering, administrative processing, specimen-related coordination, and professional services, refunds are available only in limited circumstances.
Refunds are not automatically granted because a patient changes their mind, no longer wants testing, selected the wrong test, failed to follow instructions, provided incorrect information, did not complete specimen collection, disagrees with normal or abnormal results, or is dissatisfied with the existence or content of a laboratory result.
Rios Click Labs may, in its discretion, approve a full or partial refund in limited circumstances, such as:
Once services have begun, refunds are generally not available. Services may be considered to have begun once any part of the order is reviewed, processed, released, coordinated, documented, sent to a laboratory, submitted through an electronic system, scheduled, collected, performed, resulted, interpreted, or otherwise acted upon.
Completed professional services, physician review fees, administrative processing, payment processor fees, and laboratory services are generally non-refundable unless Rios Click Labs determines otherwise or a refund is required by law.
If a specimen is rejected, insufficient, delayed, lost, or unable to be processed due to laboratory or collection issues, Rios Click Labs may offer repeat collection, substitution, credit, partial refund, full refund, or cancellation depending on the circumstances.
Refund requests must be submitted in writing to Rios Click Labs. Submission of a refund request does not guarantee approval.
You have the right to respectful treatment, privacy, confidentiality, information about your care, and the ability to ask questions regarding services provided by Rios Click Labs or Rios Healthcare Associates, PLLC.
You have the right to request information about charges, request access to your medical information as permitted by law, and communicate privacy concerns.
You are responsible for:
Medication history authorization is optional.
If you select the medication history authorization checkbox, you authorize Rios Healthcare Associates, PLLC / Rios Click Labs and its providers to obtain your prescription medication history from pharmacies, health insurers, electronic health record systems, medication-history networks, or other available medication-history sources.
You authorize this information to be stored in your medical record and used for treatment, lab-order review, safety screening, medication-related assessment, and care coordination.
You understand that medication history may be incomplete, may not include all medications, may not include over-the-counter medications, supplements, or herbal products, and may contain errors. You remain responsible for informing your healthcare providers of all medications, supplements, allergies, and relevant medical history.
You may decline medication history authorization and still complete your Rios Click Labs purchase. Declining may limit information available to providers reviewing your order.
Rios Click Labs does not provide emergency care.
Rios Click Labs services do not replace diagnosis, treatment, urgent care, emergency care, primary care, specialist care, or ongoing medical management.
Laboratory results should be interpreted in the context of symptoms, medical history, physical examination, medications, risk factors, and other clinical information.
You should not start, stop, or change medications or treatment based solely on Rios Click Labs results without consulting a qualified healthcare professional.
You understand and agree that order information, consents, laboratory orders, results, communications, payment status, and related documentation may be stored in Athena or another electronic health record or administrative system used by Rios Healthcare Associates, PLLC.
You understand that signed consent packets may be emailed to you and to Rios Click Labs / Rios Healthcare Associates, PLLC for documentation and upload into the medical record.
By checking the required boxes and typing your full legal name at checkout, you certify that:
Your electronic acknowledgment may be recorded with the order number, patient information, timestamp, IP address, browser/user-agent information, policy versions, and signed consent packet.
